Who Can Request a Birth Certificate?

Birth records are restricted under Michigan law. A birth certificate may be requested by:

  • The person named on the record
  • A parent named on the record
  • A court-appointed legal guardian

  • Valid government-issue photo ID
  • Court guardianship paperwork (if applicable)

Important Information

Birth Records Available Through Southfield

Southfield maintains birth records for individuals born at hospitals located within the city, including Ascension Providence Hospital.

Older or Special Circumstances

If you were born before 1979 and your parents were not married, or if you were adopted, your birth record may be maintained by the State of Michigan.
