All City of Southfield offices and facilities will remain closed to the public until at least Monday, February 1, 2021 or further notice in accordance with the most recent state of Michigan public health order. All essential public safety and public works services and departments continue to function as normal. Residents can still conduct most city business online and through City Hall’s main drop box 24/7 as well as via phone during the City Hall regular business hours, Monday-Friday from 8 a.m.- 5 p.m.

The Clerk’s Office also serves as the Local Registrar for all birth and death records for those born or deceased at Providence Hospital or within the City boundaries. Birth and death certificates may be obtained through the mail by completing the form available on this page. Only the parents listed on the birth record may obtain a certified copy, or the child of at least 15 years of age. A legal guardian may obtain a certified copy of a birth record when presenting current valid guardianship papers in person. Photo identification is required for all requests.
Mail-in requests must include: child’s name and date of birth; mother’s name, maiden name and date of birth; father’s name and date of birth; mailing address to send the birth/death record; phone number with area code; number of copies requested. Photo ID of one parent must be included with all mail-in requests.
The fee for all birth and death certificates is $15 for one copy; $5 for each additional copy. No personal checks are accepted – money orders payable to the City of Southfield are accepted for mail-in requests. Cash payment is accepted for in-person orders.
Certified copies of Death Certificates are available for events that occurred in the City of Southfield. Requests for Death Certificates must be made in writing, and include the full name of decedent and date of death; name, mailing address and phone number to send the death record; number of copies requested and applicable fee.
