Pursuant to the Small Wireless Communications Facilities Deployment Act (Public Act 365: Sections 460.1301 through 460.1339), this permit application submittal form has been reviewed by the City's Engineering and Legal Departments and is required for Small Cell Wireless Facilities Permit Applications within the Public Rights-of-Way.
